or Subrogation
Certificates of Insurance and Waivers of Subrogation are requested and issued through
our Worker’s Compensation department.
Certificate of Insurance
To request a certificate of Insurance for an employee, please complete the Certificate of
Insurance Request Form and e-mail to In the �comments or
special requests’ portion of the form you can write the name of the employee requesting
the Certificate of Insurance along with his or her fax number, e-mail address or home
address and AEG will forward a copy of the insurance policy to said employee.
Waiver of Subrogation
A Waiver of Subrogation may be applied for when requested by the client. This form
should be completed and sent via email to It is very
important in the �job location and project details’ portion of the form to be as detailed as
possible, including the specific address of the job location and project details. If the
request is granted, there is a fee which will be added to your next payroll invoice.
Waiver of Subrogation fees may vary but you will be notified of the incurred costs upon
your request.
