How to Complete the Autism Spectrum Disorder Supplement

Follow login procedures for the ARD Forms application.
Screen 1* Student Information:
1. Verify that the correct student record is accessed.
2. Click
in the Screen: field. Scroll down and click on the Parent Training Needs
Assessment (1) screen name. The 99* Parent Training Needs Assessment (1) screen
will appear.

Screen 99* Parent Training Needs Assessment (1):
3. Verify that the If checked, print with the Full ARD with Supplements report box is
checked. If the box is blank, click on
with your mouse and a check will appear.
4. Click
in the Parent Information: A. field and type in the appropriate information.
5. Repeat step 4 for the Parent Information: B., C., and D. fields.
6. Click
in the Parent Information: E. field next to Yes or No to answer the question,
Has the parent/family received parent training?
7. Click
at the top of the page.
8. Click
at the top of the page. The 100* In-Home Training Needs Assessment (2)
screen will appear.

in the School Information: A. field and type in the appropriate information.
10. Click
in the School Information: B. field and type in the appropriate information.
11. Click
in the School Information: C. field next to Yes or No to answer the question,
Has the student received in-home/community training within the last year?
12. Click
at the top of the page. The 101* In-Home Training Needs Assessment (2)
screen will appear.
Screen 101* Parent Training Needs Supplement (1):
13. Verify that the If checked, print with the Full ARD with Supplements report box is
checked. If the box is blank, click on
with your mouse and a check will appear.
14. Enter the date of the ARD in the Date: field. You can directly type in the date
(MM/DD/YYYY) or use the calendar function by clicking on
.

15. Click
in the 1. Goal/Objective: and 2. Goal/Objective: fields. Type the goal and
objectives in the text pop-up screens.
16. Click
and select the appropriate number of sessions, or click on NEW and type in the
number. Click in the Sessions per field. If Day, Week, or Month are not applicable,
leave blank.
17. Click
and select the appropriate number of minutes per session, or click on NEW and
type in the number.
18. Click
and select the appropriate duration, or click on NEW and type in the number.
Click
in the Duration (For how long?) field. If Day, Weeks, or Months are not
applicable, leave blank.
19. Click
if the parent received a copy of the calendar for parent support meeting and
structured parent training.
20. Type in the total number of training hours in the Total Parent/Family Training Time
field. Click
at the top of the page.
21. Click
at the top of the page. The 102* In-Home Training Supplement (2)
screen will appear.

22. Enter the date of the ARD in the Date: field. You can directly type in the date
(MM/DD/YYYY) or use the calendar function by clicking on

23. Click
in the 1. Goal/Objective: and 2. Goal/Objective: fields. Type the goal and
objectives in the text pop-up screens.
24. Click
and select the appropriate number of sessions, or click on NEW and type in the
number. The Sessions per fields are Read Only.
25. Click
and select the appropriate number of minutes in the Minutes per Session field,
or click on NEW and type in the number.
26. Click
and select the appropriate duration, or click on NEW and type in the number.

Schedule of Services (1) screen will appear.

32. Enter the date of the ARD in the Date of ARD: field. You can directly type in the date

under the Extended Educational Programming heading. The
Extended Educational Programming pop-up window will appear.

next to the appropriate Extended Educational Programming
option(s). The selected text will appear in the text box.

36. Repeat steps 33-35 under the Daily Schedule heading.

37. Enter the student’s schedule in the Daily Schedule table. Type directly into the Time and
Activity fields. Click
in the Location fields and select the appropriate location.
38. To clear all fields in the Daily Schedule table, click

42. Click next to the appropriate statement under the Suitable Staff-To-Student Ratio
heading.
43. Repeat steps 33-35 under the Suitable Staff-To-Student Ratio heading.
44. If a particular staff-to-student ratio is needed, complete the table by typing directly into
the table fields.
